VITAL RECORDS
Birth and Death records are available from the Local Registrar of the municipality where the event occurred. Marriage records are available from the Town or City Clerk of the municipality where the Marriage License was issued.
All Applications or requests for Vital Records/genealogy information must include the appropriate fees. Personal checks must clear before the record is released. The fee for a birth, death, or marriage record is $10 per record.
Birth Records
*Who is elibible to obtain a birth certificate copy?
The person named on the birth certificate
A parent of the person named on the birth certificate (requesting parent’s name must also be listed on the birth certificate)
A spouse, child, or other persons ONLY by order of a New York State Court
Identification Requirements – The application must be submitted with copies of either A or B below:
A. One of the following items of valid photo ID:
– Driver License
– State-issued non-driver photo ID
– Passport
– U.S. Military issued photo-ID
B. Tow (2) of the following showing the applicant’s name and address:
– Utility bill
– Letter from a government agency dated within the last six (6) months
Death Records
Who is eligible to obtain a death certificate copy?
The spouse, parent, or child of the deceased
Other persons who have a documented lawful right or claim, a documented medical need, or with a New York State Court Order
Identification Requirements – application must be submitted with copies of either A or B:
A. One of the following forms of valid photo ID
– Driver License
– State-issued non-driver photo ID
Passport
– U.S. Military issued photo-ID
B. Two (2) of the following showing the applicant’s name and address:
– Utility bill
– Letter from a government agency dated within the last six (6) months
